South Carolina WIC’s Nutrition Risk Code Reference Tool

South Carolina WIC developed this tool to help their nutrition staff understand the health-related issues of each nutrition risk code, how to further assess a participant assigned a particular risk code, and what interventions are needed to mitigate the risk(s) for their participants.

While each WIC State agency established their own nutrition assessment protocols and how they approach further assessment and follow up of certain risk codes, State agencies may choose to use South Carolina WIC’s training as one example when developing or updating their own training.  Appendix A in VENA Guidance also provides a helpful guide as well.
